Furthermore, the undersigned contact person for the named entry and its participants acknowledges that he/she is aware of risks associated with participation in the St. Patrick’s Day Parade sponsored by the St. Patrick Society of Peoria. The undersigned contact person for the named entry and/or any of its participants agrees to waive, release and discharge the St. Patrick Society of Peoria, the City of Peoria, the County of Peoria, and the State of Illinois from any and all liabilities for damages, death, personal injury, or property damage which the named entry and/or any of its participants may have, or which may hereafter occur to the named entry and/or any of its participants in whole or in part during the assembly of the parade line-up, the movement of the parade along the parade route, and the leaving of the parade route. In agreeing to be a participant in the St. Patrick’s Day Parade sponsored by the St. Patrick Society of Peoria, the undersigned contact person for the named entry and its participants hereby agrees that photos taken during the parade assembly, movement of the parade along the parade route and the leaving of the parade route may be used by the St. Patrick Society of Peoria in its publications, promotions, advertising or otherwise, without any further approval whatsoever. *You will now be asked for a Digital Signature.
